In an Age-Friendly Health System, every older adult gets the best care possible, is not harmed by health care, and is highly satisfied with the care they receive.

The intention of CATCH-ON Community Health is to bring together the key elements from evidence-based programs for managing multiple chronic conditions among older adults in and around primary care and create linkages to community aging resources.

Building on the work of a large and growing community, the Agency for Healthcare Research and Quality (AHRQ) defines a medical home not simply as a place but as a model of the organization of primary care that delivers the core functions of primary health care.

Primary care practices are achieving this quadruple aim by creating more effective practice teams and becoming certified as Patient-Centered Medical Homes.
